Display agreement checkbox

To display agreement checkbox, proceed as follows:

1. Open ‘My Chart Designs’.
2. Choose your design and click on it.
3. Click on ‘Birth submission form’.
4. Click on ‘Settings’.
5. Click on ‘Display agreement checkbox.
6. Save.
